Complex processes can provide the best opportunity to improve the experience of service users, the quality of service and value for money. Simple changes at clinical-level with skilful facilitation can lead to major improvements. Our experience of delivering these changes includes:
- A comparison study of acute and community COPD services against national benchmarks and a review of interventions that reduce hospital admission.
- Development of benchmark standards and measures for continuing care assessments; reviews of 24 PCTs; feedback and change planning with clinicians and social workers.
- The redesign of stroke rehabilitation pathways and discharge planning processes.
- Development of a minimum dataset for capturing, screening and assessing activity for NHS Continuing Healthcare across a large health economy, establishing data flows and management reporting.
- Change programme management to establish new NHS organisations and dissolve others.
