Continuing Healthcare Assessment - Co-ordinator Development Programme
Overview
This one day course explores the role of co-ordination, management of the full consideration process and the standards required to deliver robust multi-disciplinary, comprehensive assessments and sound MDT recommendations.
Learning Outcomes
Participants will have a broad range of learning outcomes including:
- an overview of statutory duties,policy history, the national framework guidance and tools
- the assessment pathway and role of the assessment coordinator;
- ensuring patient and family engagement;
- reviewing the quality of specialist assessments
- facilitating the work of the MDT
- chairing the MDT meeting, critically appraising needs evidence and using the DST
- an understanding of dispute resolution processes.
Target Audience
This course is suitable for a wide range of roles including care managers and senior social workers, community case managers, lead clinicians, continuing healthcare and funded-nursing care assessors, and discharge planning teams. The course is suitable for participants from the public sector and not-for-profit organisations, as well as the independent sector.
